Participants must have completed a 21KM trail run in 2025.

Steps:

1.Upload your proof of completion (screenshot or certificate)

to this Google Drive folder: [ 5OKM PROOF OF COMPLETION ]

2. Rename your file using this format:

Lastname_Firstname (e.g., DelaCruz_Juan)

3. In the form, provide the file name of your uploaded proof in the Upload proof of completion field.
